**PLAYER NUMBER REQUEST - You can request up to four different player numbers in this section.  **However, if your requested numbers are not available **YOU WILL BE AUTOMATICALLY ASSIGNED the next available number or a flip of one of your number selections.  As per club mandate, we will not be contacting parents for available number selections if your requested numbers are not available.  **#16 or #69 IS NOT AVAILABLE FOR ANY PLAYER IN THE CLUB.

**OUR STAFF IS NOT RESPONSIBLE TO SEND AVAILABLE NUMBERS FOR DIVISIONS SO PLEASE DO NOT REQUEST.

**EACH DIVISION REGARDLES IF IT HAS MULTIPLE  TEAMS WITHIN A DIVISION CAN ONLY HAVE **ONE NUMBER ASSIGNMENT PER PLAYER.  This is due to possible club mandated player movement within teams in a certain division right up and into the beginning of the season.

**ALL TEAMS PLAYERS AND GOALIES.   Game uniform package includes black and gold game jerseys and socks.  Please make sure to fill out the following mandatory items...

1.  Full player registration name (e.g.  MICHAEL SMITH)

2.  Jersey name bar (e.g. SMITH)

3.  Team level you are playing this season (e.g. U11)

**AFTER AUGUST 20TH there will be a $25 charge for rush orders as jerseys will need to be manually printed with names/numbers in house at an extra cost to be ready before the season begins.
